  • Implanted devices: Pacemaker/ICD brand/model, implant date; MRI compatibility if known.

  • Medications: list key cardiac meds (beta-blockers, ACE inhibitors, nitrates, anti-arrhythmics).

  • Anticoagulation/antiplatelets: drug name and dose (e.g., apixaban 5 mg bd; aspirin 100 mg).

  • Allergies: e.g., contrast dye, antibiotics.

  • Cardiologist & GP: names and best contact numbers.

  • Other notes: exertional limits, preferred hospital, recent procedures.
