Keep it concise and practical for emergencies:

  • Diagnosis & seizure type: e.g., focal impaired-awareness, generalised tonic-clonic, absence seizures.

  • Frequency/pattern: Typical duration, clustering, and recovery expectations (post-ictal confusion/sleep).

  • Rescue medication: Name, dose and route (e.g., buccal midazolam 10 mg, nasal midazolam 5 mg; diazepam rectal gel). Note where it’s kept.

  • Triggers & precautions: Sleep deprivation, missed meds, flashing lights, illness, alcohol—anything responders or family should consider.

  • Current medications: Anti-seizure drugs with doses (lamotrigine, levetiracetam, valproate, etc.).

  • Allergies/adverse reactions: To medicines or adhesives.

  • Safety notes: Head protection needs, airway/positioning reminders, when to call an ambulance if this isn’t already one.

  • Contacts & clinicians: Primary carer/family, neurologist, GP.

Use clear, readable language—no abbreviations that only your specialist would know. You can add supportive notes (e.g., “after a seizure I’m usually confused for ~30 minutes—please call my partner”).
