NDIS funding summary

Registered NDIS Provider

MyQRMed is a registered NDIS Provider and can support plan-managed, NDIA-managed and self-managed order pathways where appropriate.

Core — Consumables

MyQRMed products may be considered under Core — Consumables where they relate to the participant's disability support needs, plan goals and available budget.

Low Cost Assistive Technology

MyQRMed QR medical IDs are low cost assistive technology products that may support safety, communication and personal care needs.

No subscription

MyQRMed is a one-time product purchase. The QR medical profile is included with no yearly membership or subscription fee.

Who MyQRMed may help under the NDIS

A MyQRMed QR medical ID may be useful where a participant needs support to communicate important health, safety or support information across daily life, appointments, community access, transport, school, work or emergencies.

  • Difficulty communicating health or support needs clearly
  • Limited or fluctuating speech
  • Memory, recall or cognitive challenges
  • Complex medication or allergy information
  • Communication needs, sensory needs or support preferences
  • Disability-related risks when away from usual supports
  • Multiple support workers, providers, carers or environments
  • A need for consistent emergency contact and care information

The QR profile can hold selected information such as conditions, medications, allergies, emergency contacts, communication preferences, support notes, GP or care team details and additional information chosen by the profile owner or authorised person.

Why clear information matters

For many people living with disability, important health and support information moves across home, community, therapy, transport, education, work and medical settings.

When key details are hard to communicate or not immediately available, support can become less consistent and more stressful for the participant and the people assisting them.

A MyQRMed QR medical ID can help by making selected information easier to access, including conditions, medications, allergies, emergency contacts, communication preferences and support notes.

Communication and continuity

  • Communication gaps are a recognised safety and continuity issue across health and support settings.
  • Written, consistent information can help reduce confusion when different people are involved in someone's care or support.
  • MyQRMed does not replace clinical care or emergency services; it is a practical assistive technology tool to support clearer information access.

How a QR medical ID may support NDIS goals

Safety and personal care

Makes selected safety information easier to access, including allergies, medications, conditions and emergency contacts.

Communication

Provides written information when a participant cannot always explain their needs clearly.

Independence

Helps the participant carry key information across daily activities, appointments and community access.

Continuity of support

Supports more consistent information across workers, carers, providers and settings.

Flexible updates

The QR profile can be updated when medications, allergies, contacts or support needs change.

How NDIS ordering works

There are three common NDIS ordering pathways. The right pathway depends on how the participant's plan is managed.

Plan managed

Submit the NDIS order request and nominate your plan manager. MyQRMed sends the invoice to the plan manager. The product is dispatched once payment is received.

NDIA managed

Submit the NDIS order request with the required participant details. As a registered NDIS Provider, MyQRMed can support the NDIA-managed process where appropriate.

Self managed

Self-managed participants can order and receive a tax invoice or receipt for their own claiming and records.

1. Choose your product

Select the bracelet, necklace, dog tag, watch band ID or ID kit that suits the participant.

2. Submit the NDIS order request

Provide the participant and funding details needed for invoicing or claim processing.

3. We invoice or process the claim

MyQRMed prepares the invoice or claim information for the relevant pathway.

4. Your order is dispatched

The product is posted once payment or claim processing is complete.

Pre-written NDIS justification

NDIS justification

This MyQRMed QR medical ID is low cost assistive technology intended to support the participant's disability-related safety, communication and personal care needs. The product links to an updateable QR medical profile that can show selected information such as conditions, medications, allergies, emergency contacts, communication preferences and support notes. This may help carers, support workers, responders or other relevant people access important information when the participant cannot explain it clearly.

The product carries a permanently engraved QR code linked to an online profile controlled by the participant or authorised account holder. For this participant, the support may assist with continuity of information across daily life, appointments, community settings and emergencies where health, safety or support details may need to be communicated clearly. The support is low cost and low risk, and is intended to complement existing supports, clinical care and emergency services.

Useful details to include: communication needs, relevant safety risks, medication or allergy information, emergency contact needs, multiple support settings, plan goals, and why an updateable QR profile is useful compared with fixed engraving.

Important limits

MyQRMed can help make selected information easier to access, but it does not provide emergency care, clinical assessment or medical advice. In an emergency, call 000.

NDIS funding is not guaranteed. Suitability depends on the participant's plan, goals, budget, evidence and circumstances.
